Physical Therapist Job Description
We are seeking a dedicated and compassionate Physical Therapist to provide quality care to patients in their homes.

About the Job
As a Physical Therapist, you will be responsible for assessing patients' conditions, developing personalized treatment plans, and delivering therapy to improve mobility, reduce pain, and restore function.

Responsibilities

• 
  
• Conduct comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
  
• Develop and implement treatment plans in consultation with physicians and revise as necessary.
  
• Assist in development and implementation of interdisciplinary client care plans.
  
• Treat patients to relieve pain, develop or restore functional ability, and maintain maximum performance.
  
• Teach, supervise, and counsel patients and their families in physical therapy programs.
  
• Record evaluation data, treatments, and patients' responses to therapy intervention.
  
• Participate in case conferences, staff meetings, and in-service programs.
  
  
  Requirements
  
  • 
    
  • Graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
    
  • Licensed or registered by the State.
    
  • At least one year of experience in an acute/sub-acute setting.
    
  • Home care experience and supervisory experience preferred.
